Purpose: 1) To improve the quality of after-hours telephone medicine in the Extended Care and Rehabilitation Center (ECRC) of the Durham VA; 2) To involve nurses and physicians in a collaborative effort to improve this aspect of patient care; 3) To identify specific elements of successful vs. unsuccessful telephone medicine encounters in our facility; and 4) To identify potential barriers to achieving best practice.
